Disclaimer, please read carefully. By completing and submitting the Girevoy Sport Union’s PAR-Q, you are confirming you have read and agreed the disclaimer. All completed questionnaires will be kept confidential and only shared with the competition host/venue as required.

Please note you are not obliged to share your personal data with us (the Girevoy Sport Union & the Competition hosts) on this form however if you choose not to we won’t be able to register your entry to the competition.

You should always consult your physician or other healthcare provider before taking part in a competitive sport.

I understand that there is a risk of injury associated with participating in any competition ran by the Girevoy Sport Union (GSU) and/or the relevant competition host. I have answered all questions truthfully and to the best of my knowledge and I will inform the host and GSU of any change in my health.

I hereby assume full responsibility for any and all injuries, losses and damages that I incur while attending, exercising or participating in any Girevoy Sport Union competition. I hereby waive all claims against the competition hosts, Girevoy Sport Union, its committee members, or partners of individually or otherwise, for any and all injuries, claims or damages that I might incur.

All athletes have the right to compete in sport knowing that they, and their competitors, are clean. The Girevoy Sport Union believes in clean sport and work in partnership with UK Anti-Doping (UKAD) to ensure that the integrity of our sport is protected. The use of performance-enhancing drugs and other doping behaviour severely damages the legitimacy of sport and undermines the integrity of clean athletes. The anti-doping rules for Girevoy Sport Union are consistent with the World Anti-Doping Code (2015 Code), which governs anti-doping internationally. Please see our Anti Doping policy before answering for more details or if you are unsure as to which answer to select.
Images may be captured during the competition using film photography, digital photography, video or other medium and may be used by The Girevoy Sports Union (GSU) to promote the competition results, records set or publicise other future events on The GSU website, social media websites, promotional leaflets and brochures and other publicity material (such as internal and external newsletters). The images may also be provided to the media for publication in local or national newspapers or magazines. Where possible and practical to do so, The GSU shall seek written consent in respect of image capture. Where this is not possible for practical reasons, unless express objections are received, individuals attending this event are deemed to have given their consent by attending or remaining at the event. No image shall be used by The GSU for commercial purposes without express consent of the individual(s) captured in that image. As the data subject you would have the right to object, the right to erasure and the right to restrict processing of any image of you. The GSU takes no responsibility for the capture and use of any images taken at the event by any third party not directly engaged by The GSU to do so on its behalf and accepts no liability for the actions of such third parties.
